Here is a close-up of the form. A few interesting things stand out. First, even though MPC supplied the Cudas and Jo-Han supplied the GTXs, they are combined on this one order form. The form was to be returned to an enterprise called "Visual Services". I'm assuming this was a clearinghouse for promotional material.
Note the high price- GTXs were $1.58 each which works out to $14.82 in 2026. '69 GTXs were 58% more expensive than equivalent Dodge promos! Also note the Jo-Han GTXs were more expensive than the MPC Cudas. Could this be due to the colorful graphic box as opposed to the plain box of the Barracuda?
